The Jockey Club Home Improvement Scheme is donated by the Hong Kong Jockey Club Charities Trust to provide comprehensive, intensive and progressive rehabilitation training to meet the unique needs of the day after day, home and remote rehabilitation. To help patients return to their homes and stay in the community.

- Have transitional care and support needs
- Who just leaving the hospital
- Has rehabilitation potential
Elderly people who lack support can be given priority. Persons who have just left the hospital due to stroke, fracture/fracture surgery, chronic lung obstruction, neurological disease or chronic disease must be assessed by the centre therapist.
